The presrption drugs Permax and Dostinex are both drugs from a class of medications that are called ergot-derived dopamine receptor agonists.  These drugs are used in the treatment by doctors for Parkinson's, Restless Leg Syndrome, and in some cases migraine headaches. Other ergot-derived dopamine receptor drugs include the now banned diet combo drug Fen-Phen.  Fen-Phen if you recall was pulled in the 1990's for it cause in heart valve disease. Permax (pergolide) has been found in studies to make a patient five to seven times more likely to have leaky heart valves than those patients who use other drug therapies for Parkinson's. Dostinex (cabergoline) has only been approved for sale in the United States to treat hyperprolactinemia, a condition in which excessive amounts of the hormone prolactin enter the bloodstream due to benign tumors of the pituitary gland. It has been shown in studies that Permax and Dostinex both can interact with a receptor in the heart valve, causing the valve to overgrow and become floppy and leaky. Of the two, only Permax currently has a black box warning addressing the increased risk of heart valve problems in patients.

In a study published in the January of 2007 in the New England Journal of Medicine, it concluded that patients taking Permax or Dostinex, two drugs commonly used to treat Parkinson's, Restless Leg Syndrome, and Migraine Headaches, are four to seven times more likely to suffer from heart valve damage than patients on alternative drug therapies.  Patients who take Permax or Dostinex are four to seven times more likely to suffer damage to their heart valves than patients who did not take either of these medications. The January 4, 2007 issue of The New England Journal of Medicine (NEJM) contains articles about two case studies that provide some of the strongest evidence that treatment with the Parkinson's disease drugs Permax (pergolide) and Dostinex (cabergoline) can cause cardiac-valve dysfunction.

PERMAX: Parkinson's Drug PERMAX Increases the Risk of Cardiac Valve Regurgitation, if used long-term.

A new study was published and released in the March 2007 giving additional warnings on the drug Permax.  In the March 2007 issue of the medical news journal Archives of Neurology a new study provides further support to the theory that the long-term use of Permax (pergolide) for Parkinson's disease increases the risk of valvulopathy, primarily cardiac valve regurgitation. The newly released report is called "Cardiac Valve Regurgitation With Pergolide Compared With Nonergot Agonists in Parkinson Disease". According to the report, one of the objectives of this case-control study was to determine if cardiac valve regurgitation occurs more commonly in patients with Parkinson disease (PD) treated with Permax (pergolide) than in those treated with Mirapex and Requip, the nonergot agonists, at a comparable dose.  The study found the occurrence of cardiac valve regurgitation in 36 Permax users and 36 matched users of Mirapex or Requip. In the Permax group, the average regurgitation scores was much higher than the other two drugs.   Thus concluding that your risk is much higher.  Specifically the report showed Permax users in the study had regurgitation issues 0.83 for the aortic valve, 1.42 for the mitral valve, and 1.43 for the tricuspid valve, whereas in the Mirapex / Requip group the average regurgitation scores were 0.19 for aortic valve, 0.39 for mitral valve, and 0.19 for tricuspid valve.

Dr. Richard B. Dewey, from the University of Texas Southwestern Medical Center in Dallas, made the following observations:
Based on these findings and the earlier findings on this matter, "we continue to advise patients who are taking pergolide to stop taking the drug if possible and, if not, to undergo yearly echocardiography looking for valve regurgitation," the authors state.  "Our practice of recommending non-ergot dopamine agonists as first-line therapy for patients needing a dopamine agonist appears to be relatively safe from the standpoint of cardiac valve function."

Permax and Dostinex: Civil Liability
